Moving away from filler-first
Filler is an excellent answer to lost structure, and it was never built to be the opening move for every change in an ageing face. The better starting question is which layer of the face has changed, and that question leads to a different kind of plan.

How filler became the first answer
For a long time the quickest response to a face that looked older was to put something back into it. A crease appeared; a syringe followed. Hyaluronic acid filler earned that place fairly. The change shows before you leave the room, the gel is broken down by the body in time, and, unlike most things injected into the face, it can be dissolved.
The trouble with a quick, forgiving tool is that it starts to be used for jobs it was never designed to do. That is all I mean by filler-first: reaching for volume before anyone has asked what has actually changed.
What filler does well
Filler is a gel made to hold its shape once it is placed. A firmer gel placed deep against the bone can rebuild the curve of a flattened cheekbone, fill out a hollow at the temple, or give a short chin the projection it lacked. A softer one can define the border of the lip. The support is there from the first minute, so the result can be judged together in the mirror rather than imagined.
Where a face has lost structure, few treatments are as precise, or as quick to show their effect. None of what follows argues against filler for that job.
What volume cannot answer
I find it helps to think of the face in layers, each changing at its own speed. Around the eye socket and beside the nose, the bone recedes a little. The deep pads of fat that hold the cheek up grow thinner. The skin over all of it holds less water, loses elasticity and begins to loosen.
Filler is an answer to the first two changes. Loosened skin is outside its reach, since it has no means of tightening anything, and it does nothing for the texture, hydration or thickness of the skin itself.
Asked to do those jobs anyway, volume produces the looks people most dread: a lower face that seems heavier instead of lifted, cheeks pushed forward to prop up skin that needed firming, a face that passes at rest and looks wrong the moment it smiles. The product has rarely failed in those faces. It has been placed to solve a problem it cannot solve.
The alternatives, and how much is known about each
The treatments that change this picture are not one alternative but several, each aimed at a different layer. Most share one quality: they work by prompting a response from your own tissue, or by acting within the skin, so their change arrives over weeks and months. Here is what each is designed to do, and how firm the ground under each claim is.
Biostimulators, for support that builds gradually
Sculptra and Juläine PLLA are made of poly-L-lactic acid; HarmonyCa carries microspheres of calcium hydroxyapatite in a hyaluronic acid gel. Each leaves fine particles in the tissue that are designed to prompt fresh collagen while the body slowly absorbs them. How well that is established differs from one to the next. With Sculptra, new collagen has been seen in biopsies of treated skin; for HarmonyCa, the research so far comes from animal work and small patient groups. At ART Clinic, Juläine is used for the folds running from nose to mouth, the use its instructions describe. Profhilo, the fourth, is a concentrated hyaluronic acid that disperses through thin skin, which its maker designed to help both hydration and laxity; that stated purpose, not proof of new collagen, is what we credit it with.
Skin boosters, for dehydration within the skin
Each of the four skin boosters delivers small amounts of hyaluronic acid into the dermis, the layer under the surface, where it draws in water. They add no volume and leave your features untouched. What they can improve is how hydrated and smooth the skin is, and how visible its fine lines are, until the body has broken the hyaluronic acid down.
Polynucleotides, for thin or crepey skin
Polynucleotides are purified chains of fish DNA, injected into the skin and designed to help fibroblasts, which make collagen and elastin. The studies so far point the right way and are mostly small, so we offer them to improve skin quality and claim nothing beyond it.
Energy devices, for skin that has loosened
XERF passes radiofrequency energy through the surface of the skin, where it becomes heat deeper in the tissue, with no needles. Endolift® approaches from the other side: a fine laser fibre is placed beneath the skin and delivers its energy there. Both aim for two things, some contraction of the collagen already present at the time of treatment and new collagen forming over the following months. XERF is suited to loosening in its early stages; Endolift reaches further, for early to moderate laxity. Skin goes on ageing after either one, so this article quotes no duration for how long the firming holds.
A word on "regenerative"
Regenerative is a fair description of treatments that work through your tissue's own response. It is a misleading one if it suggests time running backwards. The response differs between people, takes months to judge, and rests on stronger evidence for some products than others, which is why each one above is described by what it has been shown or designed to do, and no more.
Where filler still belongs
Moving away from filler-first does not make filler a lesser treatment. It makes it a specific one, and in its own territory it remains the right choice: flattened cheekbones, hollow temples, a chin or jaw angle short of projection, a hollow under the eye in otherwise healthy skin. It is often the answer when an occasion is close and the change needs to be visible in time. And it regularly earns a place beside the slower treatments, because a face that has loosened has usually lost some support as well.
If you come to ART Clinic wanting filler, that is a perfectly good way into the conversation. The assessment shows where it will do the most for your face, and what could work alongside it.
How ART Clinic puts a plan in order
Once the changed layers are clear, the order of treatment matters almost as much as the choice. These are the principles a combined plan here follows.
i
Layers before lines.
Bone, fat, skin quality and laxity are each examined, at rest and as you speak and smile, and the plan is written against the layers that have changed, not only the crease that first caught your eye.
ii
The slowest treatments go first.
Tightening and collagen-prompting treatments take months to show, so they usually begin early, and their months of change run while the rest of the plan takes shape.
iii
Skin quality runs alongside.
Boosters and polynucleotides act within the skin rather than beneath it, so they fit beside most of the plan, with sessions spaced so that nothing crowds anything else.
iv
Structure where it is still missing.
Filler comes in where support is still lacking once the slower work has shown what it can do, deep support before surface lines, unless a date means it should come first.
v
Every stage reviewed before the next.
Each step is judged once it has settled, and what follows is agreed with you then, at a pace that suits you.
What this order aims for is a face that looks rested and well supported, with no single treatment anyone could point to.
If you already have filler
Plenty of people reading this have had filler, and some now feel unsure about it. Hyaluronic acid filler can still be present in the tissue when it no longer shows, so any new plan begins by finding what remains and where it now sits. From that point it can be worked around, topped up where it is doing its job, or dissolved, and the treatments above can join it. If you know the name of the product, mention it; if you don't, the assessment still starts from what can be seen and felt.
Bring the change you've noticed
A consultation at ART Clinic starts from what you have seen changing and ends with a plan set out in order: which treatments, for which layer, and when each begins. Filler may well be one of them. You won't be sold to, and you won't be pushed.
