How ART Clinic decides what to recommend

Every plan at ART Clinic is reasoned out from the same few questions, asked in the same order. Here they are, with a worked example, and an explanation of why the plan you leave with can have a different shape from the one you pictured on the way in.

Where a recommendation starts

People often expect a clinic's advice to begin with its treatments. Ours begins with you: the change you have noticed, what it stops you enjoying, and what you would like to be different. Plenty of people also arrive with a treatment already in mind, because a friend had it or they have had it before, and that is a perfectly good place to begin. It tells me what you are hoping for, and it is very often where the plan ends up too.

What a recommendation cannot start from is the product. A treatment is only right in relation to the face it is for, and the reasoning that gets from one to the other is what this article is about.

The four questions behind every plan

What has changed, and in which layer?

A face alters at several depths at once: the bone, the fat that cushions it, the skin's thickness and firmness, the muscles that move it. The first job is to find which of those layers accounts for what you see, at rest and as you talk.

What would make the clearest difference?

Usually one or two changes do most of the work in how a face reads. Treating those first gives you the most visible result for each appointment, and it keeps a first plan compact.

What matters to you, and by when?

A wedding in six weeks, a preference for no needles, a budget, a busy season at work: each of these legitimately shapes a plan. Two treatments can reach a similar result by different routes, and your circumstances decide which route suits you.

In what order?

Some treatments show their change in days, others over months. Beginning with the slow ones lets them build quietly while the quicker steps are timed around them.

One concern, three plans

Three people sit down and say the same sentence: "My jawline has gone soft." These three are illustrations, drawn from the kind of faces we see, and not patients. What each of them leaves with is different, because the reason behind that sentence is different each time.

"My jawline has gone soft."

i

Firm support, loosening skin.

What we find: the bone and deep support along the jaw are in good order, but the skin over them has begun to slacken, blurring the line below the ear.

The plan: a tightening treatment. XERF, from the surface with no needles, where the loosening is only starting; Endolift®, from beneath the skin, where it has gone further or early jowls have formed.

ii

Good skin, lost structure.

What we find: the skin is still firm, but the chin sits back and the angle of the jaw has lost its projection, so the lower face has no edge to hold.

The plan: dermal filler placed deep at the chin and along the angle of the jaw, rebuilding the frame the skin rests on, with the change visible the same day.

iii

Thinning skin, early loss of contour.

What we find: the skin along the jaw is thinner than it was and the contour has begun to soften, with nothing yet sagging.

The plan: a biostimulator such as HarmonyCa, which lifts a little straight away and is designed to prompt fresh collagen over the months that follow.

Three identical sentences; three different plans. And often a face shows two of these at once, so the plan combines them.

When the plan looks different from the one you pictured

Most people leave with the treatment they came to ask about somewhere in their plan. When the plan surprises them, it is almost always for one of three reasons, and each of them is there to make the result better.

The change has more than one cause. If what bothers you comes from two layers, treating one of them gets you half a result. So the plan may add a second treatment to the one you had in mind, and it will say what each is doing.

The order changes the outcome. A treatment that works through your own collagen is slow, and filler or anti-wrinkle injections are quick. Put the quick one first and the slow one arrives into a face that has already changed; start with the slow one and each step can be measured against the last. So your plan may begin somewhere other than where you expected.

A first step is sized to learn from. A first dose of anti-wrinkle injections, or a first syringe of filler, is set to show how your face responds, and refined at your review. People sometimes read that as doing less. In practice it is how a result ends up exactly right, with nothing to undo.

Your part in the decision

A recommendation from us comes as a set of options, each with what it will change, how long it takes to show, what it involves and what it costs. Where there is a quicker route and a slower one, you see both. Then the plan is built with you, around what you choose, and nothing starts until you are ready.

You won't be sold to, and you won't be pushed. You are also welcome to disagree. If you would rather start with the treatment you came in for, we will plan that treatment as well as it can be done, and explain how the rest could follow later if you want it to.

How a plan changes after it is made

A plan is a starting position, not a contract. At each review we look at what the last step achieved and decide the next one together: continuing, pausing, or adding something for a change you have since noticed. Treatments that have finished their work come off the plan, and that is a sign of it working.

Over years, the same face will need different things, and the questions in this article are simply asked again. That is what keeps a long-standing patient's face looking like their own.

Bring what you have in mind

Whether you arrive with a change you would like to understand or a treatment you already know you want, the consultation is where your plan is reasoned out with you, step by step, and set down so you can take it home.
